These MUST be received before the day of your appointment.
At East Scottsdale Medical Care, we make every effort to run on time. To ensure that we have a prompt check in, we request that new patients or any patient with a change in their insurance or demographics arrive 15 minutes prior to their appointment time. We ask all other patients to arrive 5 minutes before their appointment time to allow for sign-in and collection of your copayment. We value your time and will do our best to ensure that you are seen promptly.
Do I Need To Fast For My Blood Work?
If you are required to fast this means consuming no food or liquids, other than water, for nine to 12 hours before the test. You will be required to fast if you are checking your cholesterol or sugars.
What Do I Need To Do For My Upcoming Physical, Well Woman Exam or Pre-Operative Clearance?
Physical/Well Woman Exam: We do ask that you are well hydrated for your appointment, so please drink plenty of water during that time starting the day prior, and take all of your regularly prescribed medications at their usual times including blood pressure medication. No lotions, oils or perfumes.
Pre-Operative Surgery Clearance: Please bring any paperwork provided to you by your surgeon. We do ask that you are well hydrated for your appointment, so please drink plenty of water during that time starting the day prior, and take all of your regularly prescribed medications at their usual times including blood pressure medication. If you require an EKG please no lotions, oils or perfumes.
School Physical / Well Child-Check: Well child checks are recommended yearly all the way through high school.
